C-DOWN + R and press RIGHT, UP, LEFT, DOWN when the level number
is displayed.

At the Kemco logo, hold L + R + C-UP and press C-RIGHT, C-LEFT, B.
Then, a "Super Hard" difficulty setting will be available.
